Anselmo ''Sam'' Hernandez, 78, died Tuesday, June 29, 2004 at Parkview Memorial Hospital. Born in Los Angeles, CA, he was employed with Irmscher & Sons Construction for 35 years and was a member of Local #213 and Most Precious Blood Catholic Church. Survivors include his wife, Mary of Fort Wayne; son, John of Chicago, IL; daughters, Carmen Lewis and Vicky Hernandez, both of Fort Wayne, Delia Falk of Kendallville and Steve Hernandez and Mona Hunley, both of Fort Wayne; brother, Juan Hernandez of Monterrey, Mexico; sisters, Ines Chaires of Monterrey, Mexico and Irma Chaves of Zacatecas, Mexico; 12 grandchildren and 8 great-grandchildren. Mass of Christian Burial 10:30am Friday at Most Precious Blood Catholic Church, 1515 Barthold Street. Prayers at 9:45am Friday at Mungovan & Sons Memorial Chapel, 2114 S. Calhoun Street.
